Abigail House for Nursing and Rehabilitation

Discharge Planning

Prior to a resident’s discharge from our facility, our rehabilitative professionals make sure they’re ready for the challenges involved with returning home or to an alternate level of care. In addition to outlining personalized exercise plans and guidelines for safely performing daily routines, our therapists will make sure each patient can manage stairs and other obstacles related to returning home. Our social workers arrange for wheelchair assistance or other assistive devices, and our nutritionists may go over dietary issues that could impact a full recovery. Our nurses will provide and review education on the resident’s condition, and if applicable, information on wound care and medication management. Recognizing the importance of continuum of care, our social workers will set up followup appointments. and work with the social services and healthcare providers available within the community.

Upon leaving Abigail House, our residents are well prepared to resume optimal functionality and independence.
